SunBridge Care and Rehabilitation - Hammond House is a 69-bed nursing facility that provides a full range of skilled nursing, therapy and rehabilitation services, including behavioral services for geripsychiatric patients. Our strong clinical orientation ensures a superior level of nursing care, while our homelike environment enhances residents’ sense of security. Our goal is to restore each patient to his or her optimal level of functioning and independence.
      We use a team approach to patient care. A group of professionals meets regularly to assess each patient’s progress. Our interdisciplinary teams include a medical director, RNs and LPNs, physical, occupational and speech therapists, social workers and psychological services specialists.
